Performance Reporting form (pdf) (Interim and Final Progress)

  • Using a Dynamic Form – This is a dynamic reporting PDF form. It can be filled out using dynamic functions that include: expanding space for text responses, ability to bold and underline text by right clicking and selecting font option, ability to add and remove rows using the “+” and “-“ buttons, and auto-calculations based on users inputs in form fields.
  • Downloading the Form – The PDF form can be downloaded and saved to your local storage device, network drive(s), or external drives – and then accessed through Adobe Reader. It will not open directly in the browser. Users will need to download the form first and then open it via Adobe. 
    • NOTE: You may need to visit the Adobe Software Compatibility to download the appropriate version of the software. There is no cost for Adobe Reader Software.
  • Submitting the Form – When submitting the form, please submit the actual dynamic form. Do not submit a flattened or scanned version of the dynamic form.
